Sciatica
Symptoms
Nerve-Related Sensations: Shooting pain down the leg, burning, tingling, numbness, and electric or shock-like sensations.
Pain Locations: Deep aching in the buttock, or pain traveling into the thigh, calf, foot, or toes.
Functional Impact & Variations: Weakness in the leg or foot, difficulty sitting or standing comfortably, and constant shifting to find relief. (Note: Some patients experience severe leg pain with little to no back discomfort).
Possible Causes
Lumbar Disc Irritation: A bulging or herniated disc can create inflammation and mechanical pressure around a spinal nerve root.
Mechanical & Structural Stress: Changes in joint function, spinal degeneration, inflammation, and narrowing around a nerve.
Soft-Tissue & Regional Factors: Sacroiliac (SI) joint dysfunction, muscular or soft-tissue irritation (such as tight muscles guarding the area), and altered movement patterns.
When Chiropractic May Help
Improving Joint Function: Gentle adjustments target lumbar joint movement and pelvic/SI mechanics to reduce mechanical stress on the irritated nerve.
Restoring Nervous System Coordination: Care addresses protective muscle-guarding and compensatory movement patterns while supporting healthier proprioceptive feedback to the nervous system.
Conservative Management: Aims to help patients with disc-related or mechanical sciatica gradually return to normal, confident movement and everyday activities without relying on invasive measures.
When Medical Evaluation Is Urgent
Patients should seek immediate medical or emergency evaluation for severe red-flag neurological symptoms, including:
New loss of bowel or bladder control, or difficulty starting/controlling urination
Numbness around the groin, genitals, or inner thighs (saddle anesthesia)
Significant or rapidly progressing leg weakness, or symptoms affecting both legs
Severe symptoms following major trauma or other rapidly worsening neurological changes
What to Expect
Detailed History: Discussing pain origin, travel path, presence of numbness/tingling/weakness, positions that aggravate it (like coughing, sneezing, or sitting), previous episodes, and past imaging.
Comprehensive Examination: Evaluating the lumbar spine, pelvis, movement patterns, strength, reflexes, sensation, orthopedic findings, and nerve tension (utilizing X-rays and/or INSiGHT scanning technology when appropriate).
Tailored Care Plans: Customizing care based on individual findings and the underlying root cause of nerve irritation, rather than just chasing the leg pain.
